On Call Service/After Hours Coverage:
If you need medical care after hours, UHP provides 24-hour, on-call service on weekdays, weekends and holidays. To reach a provider, please refer to the “Our Sites” tab on the web site and find the phone number for your health center. Give the operator your name and tell them that you are a patient at Urban Health Plan. The operator will contact the provider on call who will return your call as soon as possible. If the line is busy, please call (212) 517-1891. If you have a life-threatening emergency, call 911 for an ambulance.

Sliding Fee Scale:
Patients will not denied services based on their inability to pay. A Sliding Fee Discount Schedule is available for all patients, regardless of insurance status. The discounts are based on the current year’s Federal Poverty Guidelines. The qualifications for discounts are solely based on income and family size. More information about the Sliding Fee Discount Schedule Program is located on page 20 of the patient handbook (located under the “Patients” tab).
